Onsite health screening

1. Question: How do I make an appointment for an onsite health screening?

Answer: Your school district must agree to participate in order for you to make an appointment. From this site, click "Register to attend an onsite health screening event." You will be directed to the Health Smarts portal that has a "health screening" section.

Click the link under "Health screening" and follow the instructions to make an appointment.

2. Question: What health numbers will I receive at the onsite health screening?

Answer: The clinical tests that will be performed at the onsite health screening include the following:

3. Question: Do I need to fast in order to get my health numbers?

Answer: No. The screening technician will ask you if you have fasted so that he or she can use the correct values when evaluating your health numbers, but fasting is not required.

4. Question: Do I need to add my health numbers to the health assessment?

Answer: No, you are not required to enter your numbers in the health assessment. In fact, we recommend that you take the shorter health assessment, called the "Succeed Snapshot Questionnaire," which does not have a place for you to enter your health numbers.
