Answer: Yes, as long as you are employed by a participating SISC district and considered eligible for health benefits.
2. Question: Is my spouse eligible to receive the cash award card?
Answer: Yes, any dependents over the age of 18 and enrolled in SISC health benefits are eligible to receive the cash award card.
3. Question: How can I check the status of my cash award cards?
Answer: Click here and this will take you to the Health Smarts portal. Log in and you will see "My Account" at the top of the page. Click "My Account" and in the bottom, lower right-hand corner of the screen you will see your cash award status.
4. Question: How long after I participate in digital health coaching will I receive my award card?
Answer: You will receive your award within 3-4 weeks after completing a digital health coaching session.
5. Question: When will my cash award card expire?
Answer: All cash award cards will expire. Please check the front of your award card for the expiration date.
6. Question: How do I replace a lost or stolen cash award card?
Answer: Customer service for the cash award cards is (888) 908-3483 Monday – Friday 9:00 – 5:00 CST or (800) 798-4104 24 hours a day/7 days a week.
7. Question: How do I reload my cash award card?
Answer: If your initial award card had a value of $40 for taking the assessment, it will be automatically reloaded after you have engaged in digital health coaching.
